This Phoenix is the small town in southern Oregon, not the Arizona city, and Forever Friends Pet Cremations is listed at 100 C St building 5 there. The phone is (541) 816-7686. A building number on a short street usually means a complex of units, so ask for directions if you plan to bring your pet yourself.
It carried a 4.9 rating across 174 Google reviews when we recorded it on May 11, 2026. Google lists hours of 9 AM to 5 PM Monday through Friday, closed Saturday and Sunday. Ask what happens with a pet that dies on a Saturday, and whether your vet can hold your pet until Monday.
The Rogue Valley has more choice than a town this size would suggest. Green Acres Pet Cemetery & Crematorium is 2.4 miles away in our data and, by its name, can take burial questions as well as cremation. Rogue Valley Cremation Service is 3.1 miles away and Care Cremation Service in Medford is 4.5 miles away. Both names read as human cremation businesses, so ask whether pets are accepted before you drive over.
